Because old men are known to get off on small naked bodies covered in pints of blood?
The nudity is there for a reason. It's to MAKE the viewer feel more uncomfortable, especially in psychologically disturbing scenes. Part of the story IS the "sexual stuff", but it's to emphasize emotions and themes, not fanservice.
