I believe since the author show the flashback of yukimaru being in the hospital, he would have another attack from his illness and force to forfeit, which lets eckizen takes the victory for seigaku

huh...yukimura is probaly gonna go all out ...and try to get echizen ...so when echizen is coenered he releases a really strong attack like..better than selfa-actualization...or something like that...and then they keep on going all out until ...one of them comes out of the dust...

I think its really quite obvious that echizen and yukimura will battle until the very end, and Echizen will summon the pinnacle of perfection and win the game(through much difficulty tho').

But yukimaru had a surgery during the, i think it was, the kanto tornament. So he should be perfectly fine....so it definetly NOT be because of his health condition.
